The Howard County Local Health Improvement Coalition (HCLHIC) works to improve health outcomes and reduce health disparities in Howard County.

The HCLHIC, part of the Howard County Health Department (HCHD), would like your help in selecting solutions to improve the health of community members living in Howard County.

Solutions chosen will form part of the HCLHIC's Community Health Improvement Plan for the next three (3) years.

The survey should take approximately five (5) minutes to complete and your responses are private. Please email LHIC@howardcountymd.gov if you have questions. Complete the survey by April 11, by 4 PM.
