Description: Hosted by Office on Aging and Independence. This six-week self- management education workshop is for those living with or at risk for a chronic condition such as heart disease, diabetes, cancer, obesity, depression, chronic pain, arthritis, lung disease and any other health concerns. Caregivers and support persons are welcome, however they must also register for the course.
